I've been accepted

Registration

 

As in past years we expect ro register a full freshmen class.  In order to reserve a place in next year's freshmen class you must register on or before

March 16, 2012.  Students registering after March 16, 2012 will be admitted on a space available basis.

 

Registering is an easy process. After completing the registration forms please mail them with your registration fee to Tampa Catholic in care of  Mrs. Lynch.  If you prefer you may bring in your completed forms and register in person on March 16, 2012 from 9:00am-11:00am in the Admissions Office.
To expedite the process, please make sure you bring the following items and have completed them in advance:
  • Signed letter of acceptance
  • Completed Registration Form (PDF)
  • Completed Financial Registration Form
  • Registration fee ($575 minus $100 for registering on or before March 16th)
  • Completed F.A.C.T.S. Form if not paying tuition in full.
    • (To receive the discount you may pay by cash or check only)
  • Attach voided check to F.A.C.T.S. form if paying tuition via checking account. 
  • Completed Promotional/Media Release Form (PDF)
  • Signed Random Drug Testing Agreement (PDF)

Note: You may register only after having been accepted for admission to Tampa Catholic. 

Health Physicals
Note: The information presented here pertains to the 2012-2013 school year.

If you are an incoming freshmen or transfer student and will be playing a sport:

You will need to submit FOUR forms and your Birth Certificate.

  1. State Immunization Form (blue form shown below)
  2. State Health Form (yellow form shown below)
  3. Florida Athletic Association Physical form (2 pages; see the link below)
  4. Florida Athletic Association Consent form (see link below)
  5. A copy of your birth certificate

If you are an incoming freshmen or transfer student and will NOT be playing a sport: You will need to submit TWO forms.

  1. State Immunization Form (blue form shown below)
  2. State Health Form (yellow form shown below)

If you are a returning student and will be playing a sport:
You will need to submit TWO forms.

  1. Florida Athletic Association Physical form (2 pages; see the link below)
  2. Florida Athletic Association Consent form (see link below)

If you are a returning student and will NOT be playing a sport:
You do not need to submit any forms.

All Students New to Tampa Catholic (class of 2016 and transfer students)

Florida State Law requires that each student submit a current health/physical form and proof of immunization before they begin school.  The immunizations include the Hepatitis B-series, a second MMR, and a tetanus inoculation. It is imperative that the physical form is current.
The yellow physical form and the blue immunization form are procured at your doctor’s office and will look like these shown here.
 
These forms should be mailed to the school (4630 N. Rome Ave., Tampa, FL, 33603) by July 13th in order to allow sufficient time for processing. It will not be a problem if the physical arrives a day or two late but by state law no student may start school without first having submitted this important documentation.

Note: Catholic Schools within the Diocese of S1. Petersburg require enrolling students to submit a Florida Department of Health Certificate of Immunization as provided for in Florida Statute 1003.22 as a condition precedent to acceptance. Catholic Schools in the Diocese of St. Petersburg do not recognize a religious objection to this immunization. This policy is effective as of the 2012-2013 school year.
 
Athletes – Both New and Returning Students
If you are planning on participating in any sport at Tampa Catholic, you must ALSO submit the FHSAA Physical Evaluation and Consent forms. These forms must be dated after June 1, 2012!  It is also necessary to send a copy of your birth certificate if you have not previously participated in a sport at TC.  Give these forms directly to your fall coach or the athletic director. No student will be able to participate in a sport at TC until the forms are submitted.

Please note: The State health form cannot be substituted for the Florida High School Athletic Form; they are not interchangeable

 

School Uniforms
Dear Families, 
We are pleased to announce that your school uniforms will be available through Risse Brothers School Uniforms.
Please order uniforms for the 2012-2013 school year by JUNE 15, 2012 at the Store, by fax, mail, online or phone.
 
Store Policy:
We accept Visa, MasterCard, American Express, Discover, Cash, or Personal Check (In-state accounts only).  All orders must be paid in full at the time of ordering and before any merchandise can be shipped.  All shipped orders include a non-refundable shipping fee of $8.00.  NO COD’S.
Exchanges may be made by coming into the store, or mailing/shipping the merchandise back with a note stating what we need to do.  Alterations are the responsibility of the customer. All sizes are standard sizes.
We do not accept any merchandise for return that has been washed, worn, altered, or have names written in them, EXCEPT in the case of manufacturing defects.   PLEASE TRY ON ALL UNIFORM ITEMS BEFORE YOU WASH OR ALTER THEM.
 
Cash refunds over $50.00, and merchandise paid for by personal check, will be issued a refund check from our corporate office in New Jersey.   Please allow two (2) weeks to receive a refund.  Refunds on merchandise paid for by credit card will be issued a credit on the same credit card.
 
We accept phone orders with a credit card as long as you know your sizes.  You may also fax orders to 813-282-836 or mail to the store.  You may also order online at www.rissebrothers.com. 
 
Please place your uniform order early to avoid any delays.

Monday – Closed  (Open Monday for the months of July and August)
Tuesday, Wednesday, and Friday – 9:30 AM to 5:30 PM
Thursday – 9:30 AM to 7:00 PM
Saturday – 10:00 AM to 4:00 PM
Sunday - Closed

Fall Athletic Try-Outs

All incoming and returning students interested in athletics, cheerleading, or our Crusaderette Dance Team must be accepted, registered, and have paid their registration fee to Tampa Catholic in order to be eligible for try-outs or conditioning programs. Current students must have a minimum 2.0 cumulative grade point average. Also, students must turn in the completed FHSAA physical and consent forms to the coach by the first day of tryouts.

 

Athletic Tryouts:
Information regarding tryouts for Fall sports will be posted to the Tampa Catholic website during the summer as the details are announced.

  • Cheerleading and Crusaderette
    Try-outs are held each spring.
  • Crusaderette Dance & Twirling Team Tryouts
    The try-outs for the 2012-2012 season will be held April 23-27 in the TC gym.  There will be an informational meeting in Crusader Hall on Monday, March 19 at 6:00 pm.  For further information contact Sue Johnson at 870-0860 x539.
  • Cheerleading
    Try-outs for the 2012-2013 season will be held April 30-May 3 in the TC gym.  Try-outs are from 5-8 pm.  If you have questions contact Tami Van-Treese at 813-870-0860 ext  525.
